Geriatric Solutions is a program of not-for-profit Hospice of the Valley

Our physicians and nurse practitioners provide primary care services to people in the comfort of their own home, wherever they live. We also provide support and education to their families. Our proactive approach helps avoid unnecessary trips to the emergency room and hospitalizations. We help you navigate the complex healthcare system by coordinating care with specialists and other providers.

We deliver compassionate care to chronically ill and home-limited patients who find it difficult to get to the doctor. We understand the challenges of living with advanced illness. It is often difficult to coordinate care with a number of different providers.

After a hospital discharge, we can provide transitional care, including necessary medical equipment and palliative care for chronic illness.

We can also coordinate care with specialists in areas such as cardiology, endocrinology and nephrology, along with in-home services like podiatry, audiology and optometry. We provide assistance with referrals for home health services like wound care or physical, occupational, and speech therapies.

Worried about loved ones who can no longer manage on their own? Our no-cost service supports families seeking the right home with the right level of care.

At end of life, we help families find compassionate hospice care for their loved one.

    Review and reconciliation of medications. Consultation and coordination with a clinical pharmacist as needed.

    Home visits by clinicians with expertise in pulmonary care and dementia care.

    Specialized evaluation for patients recently discharged from the hospital to ensure successful transition home.
  • HOME SAFETY
    Assistance with in-home evaluations to prevent falls and other injuries.
  • SENIOR PLACEMENT SERVICE
    No-cost assistance finding appropriate care and living arrangements for a loved one.
  • CAREGIVER SUPPORT AND EDUCATION
    Support to help families cope with stress and understand the disease process.

WE COORDINATE

  • IN-HOME X-RAY AND LAB SERVICES
    EKG, radiology, TB tests, blood draws, urine analysis, cultures, ultrasounds and IV infusions are coordinated by our staff.
  • IN-HOME SPECIALTY SERVICES
    Care coordination for specific needs like podiatry, audiology and optometry.
  • SPECIALIST COORDINATION
    Care coordination for specialties like cardiology, endocrinology and nephrology.
  • HOME HEALTH SERVICES
    Care coordination in the home for physical therapy, occupational therapy, speech therapy, wound care, and other skilled nursing services.

SERVING YOU AT HOME
We serve home-limited adults who find it difficult to get to the doctor due to advanced illness like Alzheimer’s disease and other dementias, cancer, COPD, heart disease, kidney or liver failure and neurodegenerative diseases. We provide care in private homes, group homes, assisted living facilities and long-term care facilities.

We serve patients who have many chronic illnesses and conditions, including:
• Alzheimer’s disease and other dementias
• Cancer

• Kidney or liver failure
• Limited mobility
• Neurodegenerative diseases like multiple sclerosis
• Parkinson’s disease
• Severe arthritis

Eligibility
We accept primary insurance with Medicare, select Medicare Advantage Plans, commercial plans, as well as most secondary insurance plans. Patients may also pay privately.
