HISTORY
The Sisters of St. Joseph of Carondolet began with six women meeting in a small kitchen in LePuy, France, 1650. These six women had a common desire to grow in their love of God and serve the unmet needs of the desperately poor people in the city around them, whom they came to call their “dear neighbors.” These women with the spiritual direction of a Jesus priest, Jean Pierre Medaille, formed the first community of Sisters of St. Joseph. The sisters lived simply and openly among the people, rather than within cloistered walls as was the norm of religious women of that time.

As Sisters of St. Joseph in the 21st century, we continue to respond to the unmet and critical needs of our contemporary society and world. Our mission and vision, informed by a deep and pervasive love of God and neighbor without distinction, move us always toward ways to respond, in a spirit of unifying, reconciling love, to the needs of our local communities, the call of the Church, the people of God, and the challenges of the global community within which we live.

HEALTHCARE SERVICES FOR THE HOMELESS

  • Holistic integrated care for individuals experiencing homelessness, meeting the patient’s medical, social, emotional and spiritual needs.
  • The only medical respite program for those experiencing homelessness in the state of Arizona and is a model for the country.

Health care services for the most vulnerable individuals in our community:

MEDICAL RESPITE
Circle the City offers a recuperative medical respite model where individuals experiencing homelessness can recuperate and receive daily medical care and round the clock (24/7) nursing support. During the stay we provide integrative services to end their homelessness and connect them to support services in the community.

We care for individuals who are too vulnerable to be on the streets and gives them a safe place to heal and get back on their feet.

We offer an integrated care team, led by a primary care provider, a behavioral health consultant, a psychiatric consultant, and clinical support staff and enabling staff.

OUTPATIENT
Circle the City has two traditional fixed sites providing outpatient care to individuals.
We also have ten satellite healthcare sites embedded within other social service agencies. There we can provide health care for the most vulnerable people who are served at that site. See a list of Circle the City outpatient sites.

Circle the City’s Parsons Family Heath Center provides primary care and preventative healthcare for individuals and families experiencing homelessness. It is a state-of-the-art facility where patients can receive a range of primary health care services.

Outpatient services include:

  • Prenatal checkups
  • Well child visits
  • Immunizations
  • Behavioral health services
  • Substance abuse therapy
  • Health & wellness groups
  • Case management

We offer an integrated care team, led by a primary care provider, a behavioral health consultant, a psychiatric consultant, and clinical support staff and enabling staff.

OUTREACH
Circle the City also offers Mobile Medical Units with medical exam rooms. This allows us to bring health care services to more individuals who need it. Each mobile medical unit has a medical provider and clinical support staff.

  • Neighborhood Outreach – Circle the City offers a medical provider, behavioral health consultant, nurse, and medical coordinator who visits neighborhoods in the most need.
  • Backpack Medicine – Circle the City also offers a team of medical staff to reach out to homeless in parks and encampments.

INTEGRATED CARE
Circle the City’s approach to primary healthcare encompasses behavioral health screening and intervention as a part of everyday, routine primary care. Each care team at Circle the City includes a Behavioral Health Consultant who, together with the Primary Care Provider, offer the following interventions to assist the patient in meeting their physical and behavioral health goals:

  • Behavioral Health Screenings
  • Assessments
  • Behavior change activation
  • Self-Management education

The Behavioral Health Consultant acts as an educator to the medical staff and patient alike and often assists the Primary Care Provider in making accurate diagnosis related to a patient’s behavioral health. This team-based approach to integrated primary care removes the stigma often associated with behavioral health care and increases engagement and care plan success of the patient.
